We aim to improve and support the quality of patients' care in clinical areas by the provision of a professional and effective medicines management service, with the emphasis on the re-use and assessment of patients own medications, medicine reconciliation, patient counselling, preparation of discharge prescriptions, one stop dispensing and authorised checking. This will include 4 month rotations in a variety of clinical areas such as: Health Care of the Older Person (HCOP) & Frailty - This would include time spent on the newly opened Geriatric Assessment Unit (GAU), a specific admissions area for people older than 75 with an acute illness and the base HCOP wards. Diabetes & Endocrinology (C52) - Caring for patients with diabetes and endocrinology conditions requiring hospital care. The ward also sometimes cares for rheumatology and dermatology inpatients. Medical-Surgery patients (C53) - This ward cares for patients with medical needs whose speciality sits under the surgical division, including vascular, neurology and gastroenterology.
