The post holder will be an experienced Healthcare Assistant who will support in managing specific cohorts of patients with a range of acute and chronic medical conditions. They will work as part of the team, delivering care within their scope of practice to patients within the surgery and conducting home visits. They will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education. They will carry out baseline observations such as pulse oximetry, blood pressure, phlebotomy, BMI, temperature as directed and record findings accurately. They will support with management of chronic disease, delivering opportunistic health promotion where appropriate. The post holder may complete basis care plans. This role is initially offered on a 12 month contractual basis, further extensions to be discussed.A travel allowance will be paid for completing home visits About us Health care assistants play an important role, they work closely with GPs and practice teams to manage a caseload of patients, acting as a central point of contact to ensure appropriate support is made available to them and their carers; supporting them to understand and manage their condition and ensuring their changing needs are addressed.
