Position of the role in Claire House The post holder reports to the Young Adult Co-ordinator and is accountable overall to the Director of Clinical Services Scope of Job To act as a resource for all members of the multi-disciplinary team providing supervision, guidance, care and support To act as a contact worker demonstrating a good level of core communication skills to meet the specific needs of the child/young person and their family To assess, plan, implement and evaluate the clinical and/or emotional requirements of children/young people with complex, palliative and end of life care needs Dimensions and Limits of Authority Clinical Community Nurses (CCN) new in post will only take on the full aspects of the role post Probationary/Preceptorship period. By providing specialist nursing, end-of-life care, and emotional support, Claire House supports families through some of the toughest times of their lives. Not all families want to come to the hospice or our Liverpool site, so our team goes out to visit them in hospital or their homes and we put on regular trips for brothers and sisters to ensure they dont miss out on having fun. The Young Adult Team at Claire House children and young adult hospice are delighted to offer this unique opportunity for a Band 5 nurse to join the team. We are looking for a proactive and dynamic individual, capable of working across various teams and services within the organisation and across statutory services. The individual needs to be an excellent communicator, who can prioritise, manage their own caseload and work autonomously. Within the small young adult team we cover transition support, palliative transition and we have our own caseload of teenagers and young adults who get their own bespoke offer tailored to their needs focusing on the benefits of social interaction. This role is full-time (37.5hrs) and can be worked flexibly across 4 days, with an expectation to work flexibility over a 7-day period if needed, with an option to work at home 1 day a week; dependent on need. The main responsibilities would centre around working across social group sessions, day to day key work for the case load, support with palliative transition, attending joint clinic appointments at adult hospices, general networking and collaborative working with various teams across the NHS.
