Under the supervision of the Locality Manager, they will work autonomously and as a member of the Community Palliative Care Team influencing the delivery of ongoing patient care. Key Tasks To develop and maintain communications with patients, carers, other professionals and volunteers To work collaboratively, building relationships with other members of multidisciplinary team and outside agencies to ensure individualised care is provided in a timely and responsive way To provide advice by telephone to patients, carers and other professionals, as required from the Community Care Coordination Centre To give and receive accurate reports and changes in patients condition, linking with key clinical staff where required, to facilitate comprehensive and safe patient care To maintain accurate and contemporaneous records in all areas of clinical practice To provide a rapid response visit to a patients home when required, undertaking a full clinical assessment of need, prioritisation and delivery of care To undertake joint clinical assessments with other professionals and liaise as appropriate regarding ongoing care.
