The post holder will provide, highly specialist assessment and interventions for a caseload of patients on acute medical and surgical wards demonstrating advanced clinical reasoning skills and the ability to undertake assessment of complex patients including cognitive assessment, functional and environmental assessment, and to play an active role in discharge planning, identifying the needs of patients and making appropriate recommendations for their onward care or rehabilitation.
