The postholder will support therapists with functional assessments and rehabilitation to determine their long term needs with the aim of maximising function and independence. Assessments and interventions will be carried out in care homes across Manchester and home visits may also be required. The Crumpsal Vale Team includes social workers, nurses and therapists working together to complete comprehensive assessments and agree a discharge plan, working towards a return home wherever possible. The post holder will follow agreed protocols for ensuring appropriate and timely input and good communications with all partners to ensure continuity of care whilst respecting patient choice and autonomy. Evidence of achievement at GCSE pass level or equivalent NVQ level III in care Effective verbal communication skills. Evidence of effective, appropriate written communication. General administration and IT skills. Demonstrate the ability to organise and plan. Experience of working within a team Experience of working in a health or social care environment Commitment to assisting in the delivery of high quality patient focussed care. Ability to move and handle people with equipment with appropriate training. Willingness to work flexible hours. Must be fully vaccinated against Covid to be able to work Ability to work both on own initiative and within a team. Ability to travel between sites. Full Driving licence and access to vehicle Ability to work without direct supervision in a variety of settings.
