Our Complex Care Team works with vulnerable residents and their carers who are often experiencing a crisis in their lives. Taking a strength based approach we support them to identify ways to address their concerns and situations they are currently experiencing and to address needs and minimise risk. The resident could have addiction issues and chaotic lifestyles, self-neglect, hoarding, neuro-diverse, young adults transitioning from children to adult services and meeting the criteria for support under Care Act 2014, requiring direction from Court of Protection due to care and or accommodation or have safeguarding concerns that need to be addressed to maintain their safety and wellbeing. Following a recent internal re-structure to meet the changing demands and needs of the service, we are seeking experienced, talented, and motivated candidates for the role of Deputy Team Managers, Social Workers Level 1 and Level 2, and Social Care Assessor to help us to make our vision a reality. Complex Care team is a very supportive team who supports staff through regular structured supervision and ad hoc management support, flexible working, excellent learning opportunities often tailored to the specific needs of the team. We encourage peer learning and development and as part of CPD look at encouraging progression through the Practice Education Professional Standards (PEPS) and Best Interest Assessor Award (BIA) along with other mandatory training. Social Care Assessor An exciting opportunity has arisen to employ a Social Care Assessor. You will be expected to manage a protected caseload of less complex case work. Taking a strength base approach, you will support individuals and their carers to complete a comprehensive assessment, identify strengths and care and supports needs and how to meet the needs. You will work with other departments to commission services to support residents using direct payments, ISF or conventional care and support. At times you will be asked to support other colleagues within the team with other clients. We would particularly welcome those candidates that have British Sign Language (BSL) Level 3 minimum to support deaf clients and their carers, however this is not essential.
