To assess the individual in the most appropriate environment and to be responsible for the assessment, prescription and evaluation of specialised equipment, taking into account safe use/risk to patients and carers. To overcome barriers such as hearing loss, fear, behavioural issues, language problems in order to maximise rehabilitation potential. The team provides rehabilitation to patients in their own home & residential care. The service provides specialist community therapy assessment & treatment to improve health & independence. In addition, the service offers rehabilitation to enable earlier discharge from hospital or to prevent hospital admission. You will work within a multi professional team & hold a varied caseload providing therapy input & high-quality rehabilitation to people within their own homes & local care homes. You will assess, manage & provide treatment to individuals with complex and/or multiple needs, working closely with the MDT providing supervision/support to other staff.
