Competencies / Wider Responsibilities The post holder will themselves undertake the following and lead and support the Macmillan Cancer Care Support Co-ordinators to: Co-ordinate the care for a defined group of patients assessed by a registered practitioner as having level one care needs for risk stratified supported self-management. Triage telephone and email enquiries, using a risk assessment framework, initiating an appropriate response according to protocols and individual pathways referring complex decisions to the Team for assessment and review; some requiring tact or diplomacy, prompt action and communication. Contribute to the holistic needs assessment and the development of individuals care and support plans, including supporting with actions in line with standard operating procedures and protocols. Co-ordinate and organise appointments, assessments and investigations as needed, including fast tracking individuals back into the system if required. Organise and prioritise the designated workload according to identified needs. Make pre-planned telephone calls to patients to assess on-going needs and to enable a proactive prevention approach. Co-ordinate the handover with other teams to facilitate safe, effective seamless transition of care between services. Assist with producing patient treatment summaries. Promoting healthy lifestyle choices and risk stratified supported self-management as part of consistent and planned aftercare. Supporting the delivery of patient and carer training and education, including use of equipment.
