Do you want to be part of developing and shaping a new integrated care service in Barnet? If so, an exciting new opportunity is available for a Registered Nurse to join our newly restructured Planned Integrated Care Service. Barnet Planned service provide community Nursing services to Barnet CCG and is split into three locality teams. This post will be located in one of the Barnet localities. The post holder will work closely with the team leads/ clinicians to develop the service and maintain close working relationships with both acute and community services. We are offering the exceptional candidate the opportunity to work in the planned care services. This will give you experience in the the community nursing team. The Clinician must demonstrate good clinical skills and good understanding of working in Community services. They need to be able to evidence a good understanding of the quality agenda and demonstrate experience in collaborative working with MDT teams including social services, GPs and voluntary services.
