This service includes Urgent Community Response, Hospital at Home, Frailty Same Day Emergency Care and the front door Acute Frailty Team. Successful candidates will rotate through these services to gaming training and experience in managing older people living with frailty and Comprehensive Geriatric Assessment. They will gain entry to the trainee Advanced Clinical Practitioner programme at the Royal Surrey, where they will be supervised in their frailty training by Consultant Geriatricians towards attaining Tier 3 expertise aligned to the Frailty Core Capabilities Framework. Main duties of the job This is an exciting time to join our expanding team, as we work to implement our Ageing Well Strategy across the Guilford and Waverley healthcare system. This has supported us in developing a Frailty Same Day Emergency Care facility based in the Acute Hospital, direct conveyance to this unit by working in partnership with SECAmb, a Geriatrician of the Day model providing acute clinical triage and a Hospital at Home service, offering Hospital level care to older people with frailty in their own homes. This Tier 3 development programme sits within our Guilford and Waverley Frailty Academy, looking to standardise education and training across the health and care system. Successful candidates will become part of the Academy's Faculty, contributing to delivery of the recently acquired workforce and innovation bid of £500k. Individual training needs will be met by undertaking an MSc in Advanced Clinical Practice and a Clinical portfolio in order to achieve the level of competence required by the post. The trainee will be paid at Band 7 (minimum of 3 years) until completion of all competences deemed necessary for autonomous practice where they will then paid at Band 8a.
