UCR services work with patients in their homes across Brighton & Hove and East Sussex communities to avoid hospital admissions and support those being discharged home. Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. They deliver: A rapid response to people in the community who are experiencing acute episodes of ill-health. A supported discharge service to facilitate timely hospital discharges. Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living. We are looking for Staff Nurses to join us, so if you’re a motivated and enthusiastic nurse looking to be part of a supportive team and use your skills to provide excellent cutting-edge urgent community care, we’d love to hear from you! Main duties of the job The focus of this role is delivering high quality care in the community, working with patients towards re-enablement, self-management, and independence throughout the continuum of care. You will do this by delivering holistic assessments, planning, implementing, and reviewing care provision. You will be part of a team of colleagues supporting evidence-based care, facilitating staff learning and development. You will provide nursing input for patients including medication management, wound care, catheter care and identifying deteriorating patients. This post is an ideal opportunity for a nurse who is keen to develop both their holistic assessment skills and knowledge of urgent community response. If you are new to Sussex Community NHS Foundation Trust, you will join our preceptorship programme, through protected time and regular clinical supervision with dedicated registered professional support.
