This will involve running searches, monitoring targets and contacting patients. To support the wider PCN in developing effective working relations which maximise health outcomes for the PCN patient cohort IT and Administrative Support: Linking with the PCN operations manager and PCN administrator to ensure appropriate searches developed and kept up to date to monitor progress Using PCN dashboards to monitor progress Having a good level of understanding of EMIS, Ardens and other tools that can support highly effective working across the PCN; develop appropriate EMIS searches to support work streams as necessary PROJECT ADMINISTRATION: Newcastle North PCN is constantly looking to develop new services to meet the needs of the PCN contract and our patient population. This role will offer you the opportunity to get involved in these developments. We would like to use your organisational skills to deliver services on time, to the highest standard. Support the general administration of projects and services delivered by our PCN. Support functions covering planning, tracking, reporting, quality management and internal communication. Support the production of project performance reports for the Board. The care coordinator will assist each of the member practices within the PCN in their collaborative work. The post-holder will work closely with the Clinical Director, Digital and Transformaton Lead, Operations Manager, PCN Administrator and Practice Managers across the network. There are 3 care coordinators within the PCN, working with each GP practice. It is expected that the care coordinators will work as a team to share good practice and on occasion provide some cross cover. The main requirements of this role will be a combination of identifying and understanding specific patient cohorts, patient engagement and administrative tasks that support the smooth and effective running of the PCN. Though the role is predominantly organisational, there will be a requirement to contact patients (via text, letter or telephone) for a variety of reasons such as booking for vaccinations, arranging health screening or gathering data. As a highly motivated individual, the post holder will be expected to provide high quality co-ordination support across a range of services and projects delivered by Newcastle North PCN.
