There are 2 Part time posts available - 25 hours per week and 30 hours per week. The service operates seven days per week 17.15 - 08.30 hours and provides community nursing care in the out of hours period to people living in West Cheshire. We have a number of posts available for full and part time staff and we would love to hear from you if you are interested in working evening or night shifts, or both. The team maintains community nursing services for the patients of our Integrated Teams across West Cheshire over the twenty four hour period and works predominantly with people approaching end of life, their carers and families. The team is supported by our GP Out of Hours Service and provides an urgent response within the out of hours period. CWP provides health and care services for local people, including mental health, learning disability, community physical health and all-age disability care – including the provision of three GP surgeries.
