The post holder will be based in Cramlington Health Centre, within an established skill mixed team. Main duties of the job The Health Visiting role is to deliver the Healthy Child Programme as part of a one team approach, planning, delivering and evaluating an efficient and cost effective service for an identified population, you will be delivering the National and Local Public Health agendas in response to identified health and social need working with partner agencies in a strategic, creative and holistic way to ensure best outcomes for children, young people and families. The HV is responsible for leading and delivering the Healthy Child Programme against the 6 early years high impact areas, ensuring services are provided within the tiered model approach- building community capacity, universal, universal plus and universal partnership plus. They will have excellent interpersonal and communication skills and be able to deliver under pressure and able to work in a rapidly changing environment.
