The Advance Therapies Navigator will form an integral part of the Rheumatology multidisciplinary team approach in caring for patients diagnosed with inflammatory rheumatological disease that require treatment with advance therapies.They will provide support to the Rheumatology Consultant/Nursing team regarding the initial supply and on-going maintenance for this cohort of Rheumatological patients. The Advanced therapies Navigator will work in close unison with the clinical nurse specialists to ensure the coordination and management of patients at all points of their advanced therapies treatment and will act as a point of contact for patients. They will support the Advanced therapies advanced line, dealing with patient queries, prescription queries and triaging to the CNS team as appropriate. To ensure patients are having their blood monitored as required to enable authorisation of repeat prescriptions and liaise with patients regarding this as required. The post holder will work independently within the remit of the role whilst following the To work with Consultants and Nurse Specialists across Rheumatology and the Pharmacy team to provide administrative support to the high cost drugs processes. To support pharmacy with the early transfer of patients to biosimilar medicines To check timing and process follow-up appointments, including ensuring blood tests are up to date. To monitor patients through the patient pathway to ensure that care is provided in an efficient and timely fashion and in accordance with treatment guidelines. To ensure ongoing approval of funding of supply of medication. To establish and maintain systems and processes to monitor and report on the delivery of this growing service. To respond to telephone and face to face enquiries from patients/carers. To redirect and prioritise queries as appropriate, ensuring both efficiency andeffectiveness of service delivery. To work with pharmacy to resolve any patient-specific queries or process issues. To ensure repeat prescription requests are processed effectively. To establish and maintain databases of patients receiving these drugs to enable effective on-going monitoring of patients as well as enabling both clinical and managerial audit. To ensure correspondence with GPs and other medical professionals, both within and outside the Trust, is up to date. "}
