As an ICB, we're focused on: o improving outcomes in population health and healthcare o tackling inequalities in outcomes, experience, and access o enhancing productivity and value for money o helping the NHS support broader social and economic development Main duties of the job The post holder will act as an expert Senior Learning Disability Continuing Healthcare clinician. providing specialist clinical advice, supervision and management of a team of specialist nurse assessors. They will manage the team to ensure delivery of skilled assessment, placement, case management and monitoring of clients with a learning disability for NHS Continuing Healthcare (CHC). They will be responsible for agreeing joint funding and ensure all joint funded clients who have a diagnosis of learning disability are reviewed, as well as overseeing any CHC appeals and retrospective claims. The post will lead on developing and operationalising strong transition arrangements working with Children and Young People teams and the local authorities. The post holder will need to develop key working relationships with a variety of internal and external teams and stakeholders, including: Colleagues across Complex Care, Finance, HR, Safeguarding, Quality, Local Authorities, Adult Social Care, District nursing services, GPs,Palliative Care Service, Families, Carers, Acute Hospitals, Home Care Providers, Legal teams and Integrated Learning Disability Teams. The post holder will also have experience of managing a team of clinicians and providing clinical supervision, ideally as a clinical lead; carrying out health care needs assessments whilst working with multi-disciplinary teams; managing contentious or sensitive issues, and proven experience of financial management.
