You will visit patients in hospital, or at home, and teach them how to submit their observations for review by the clinical teams. You will be expected to liaise with Virtual Ward clinical colleagues to ensure appropriate patients are referred to the Virtual Ward and are managed effectively. You will communicate with patients/carers to ensure a smooth transition onto the Virtual Ward and successful discharge from the Virtual Ward. After required training you will support the clinical teams by collecting samples from Virtual Ward patients, in hospital or at home, when necessary. Provide clinical and non-clinical support to the multi-professional team under the direction of an appropriate registered health care professional. Deliver direct and indirect care as delegated under the direction and supervision of an appropriate registered health care professional. Support the delivery of effective health care to patients/service users by undertaking duties as identified by the multi- professional team. Undertake specified and unsupervised aspects of care and therapeutic intervention as delegated by the registered health care professional within an agreed protocol, sphere of responsibility and after completing appropriate competency- based training agreed by the Trust. Support the day to day management of the Virtual Ward equipment and stock for all pathways.
