Interviews will be held on 11/10/2024. This is a fixed term/secondment post until 31/03/2026. If you are a Hywel Dda University Health Board employee and are applying for this post as a secondment, agreement for the secondment is needed from your current manager prior to application. This is an exciting opportunity to join the multidisciplinary Optimisation and Prehabilitation Services within Hywel Dda University Health Board. The post holder will provide specialist pharmacy input to support health optimisation to patients awaiting planned orthopaedic, general surgery and gynaecological surgery. This forms part of the wider Welsh Government Planned Care Policy to support people to prevent deteriorating health, promote health behaviour and prepare treatment whilst they are waiting. Job overview: 1. Responsible for delivering, managing and developing high quality, specialist pharmacy service within Optimisation and Prehabilitationservices within Hywel Dda university health board.2. Provide specialist Pharmacy input into the Optimisation and Prehabilitation service to patients on orthopaedic, gynaecology and colorectal pathways.3. Act as an independent prescriber, undertaking, agreed clinical sessions, initiating, modifying, monitoring therapy and adjusting treatmentoptions for patients.4. Review the pharmaceutical needs of patients referred for specialist pharmacist input in the Optimisation and Prehabilitation clinics. - Plan and co-ordinate specialist pharmacy provision into the optimisation and prehabilitation service to surgical orthopaedic, gynaecologyand colorectal patients.- Review the pharmaceutical needs and provide expert advice to patients referred for specialist pharmacist input in the optimisation andprehabilitation clinic.- Document pharmaceutical plan and share the written plan with the patient, primary care and secondary care.- Deliver patient-centred care through shared decision making and ensure information communicated to patients is at a suitable level andsupports informed choice.- Work closely with the MDT: Doctors, Surgeons, Nurses, Dietitians, Physiotherapist, Occupational Therapists and the Admin Team.- Use own initiative to develop pharmacy service provision in the Optimisation and Prehabilitation Service. This includes writing andimplementing new guidelines and policies to ensure safe and optimum use of medicines. "}
