Main duties of the job We are looking for an ANP who is a team player, and committed to Primary Care . Currently we provide a mix of triage, routine phone calls and face to face appointments but are continually adapting to the changing situation. The successful candidate would do a mix of these as well as a share of home visits. We are also looking to expand the role to include an element of clinical admin such as medication reviews to support the GPs. There is the opportunity to lead in a specific area of interest. About us We are a large, friendly, dedicated practice with a determination to provide high quality clinical care for our patients, at the same time we are highly committed to looking after our doctors as well. We are situated in North East Wales on the Welsh/English border. There are good connections to nearby cities with the historical city of Chester being only 5 miles away. There are also excellent schooling options in the local area. We have a list size of 10500 patients and deliver our GMS contract over 3 sites. Our main site is Hawarden Health Centre with branch sites in Saltney and Buckley. Hawarden is located 6.7 miles from Chester and is 4 miles from the English border, we are close to motorway links giving direct access to Liverpool and Manchester. The postholder will operate within their own sphere of competence and agreed acceptable limits of practice to provide expert professional clinical advice to patients, carers and colleagues, ensuring clinical safety and excellence. The Advanced Nurse Practitioner will need to prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations in the appropriate manner. The postholder will use skills, knowledge and competencies as a qualified Advanced Nurse Practitioner in order to be responsible and accountable for managing patient caseloads for treatments, referral, admission and discharging patients as appropriate.
