West Community independence Service are looking for a Registered Occupational Therapist to join our dynamic multidisciplinary team. The service provides assessment, clinical monitoring and therapy input to promote recovery. The team promotes interprofessional working which supports the single assessment process for patients. There is a robust supervision frame and opportunities for training and development. You will need effective communication and interpersonal skills, be flexible and have a positive approach to problem solving. Having a valid driving license and access to a car is essential. Work as part of the integrated locality health and social care team delivering programmes of rehabilitation within a community setting in close collaboration with other relevant services/agencies e.g. other community and inpatient teams, social care, primary and secondary care. To manage a caseload of patients providing both occupational therapy and multidisciplinary rehabilitation interventions using evidence based, recovery focused principles. The post holder has responsibility for a caseload of patients, assessing need, formulating intervention plans, delivering interventions and evaluating outcomes. Working under the supervision of a Senior OT they must ensure that they work within the scope of their competency and that their practice meets Trust and professional standards. "}
