We are looking for someone who is friendly and enthusiastic to work as part of our experienced nursing team. Excellent communication and a team player is a requirement for the successful candidate. We have a practice population of 12,500 patients, based in the heart of Caerphilly. We are within easy access to the A470, M4, train and bus station. Car parking is free on site Primary care experience would be desirable however for the right candidate training and mentoring could be given. The practice does not follow the agenda for change. Salary will be based on experience. Main duties of the job The main duties of the role as practice nurse are listed below: Assist in and perform routine tasks related to patient care as directed by senior nursing staff and GPs Chronic Disease Management - Diabetes, COPD, Asthma, Hypertension HRT and Contraception reviews Cytology Wound care / ulcer care/Doppler INR - anticoagulation monitoring Removal of Sutures Travel Clinic - immunisations ECGs Venepuncture Routine immunisations and Childhood immunisations Following agreed clinical protocols with referral to senior nurses or GPs as appropriate About us We strive to create a friendly and supportive working environment. Provide general and specific health screening to the practice population in accordance with agreed protocols 3. Provide advice on health care and minor ailments to patients 4. Run chronic disease clinics offered by the practice 5. Liaise with Health Board staff, secondary care staff, community nursing staff and other relevant bodies to ensure an excellent quality of service for patients 6. Record accurate and timely data in patient records 7. Complete admin responsibilities on a timely and accurate basis eg. Maintaining pharmaceutical and medical supplies, stock taking, maintaining equipment, QIF data recording 8.
