This role is based out of our Weston General Hospital hub but may be required to work trustwide. The NHS@Home Band 6 nurse will: Coordinate the daily operations of the NHS@Home service. Maintain an oversight of all patients being supported under the service. Support staff members undertaking patient visits and make autonomous decisions regarding patient management plans. Undertake accurate patient assessments within in-patient and out-patient settings. Support in-patient identification and manage referrals. Work with system partners to oversee system-wide service capacity. Assess patients and make comprehensive reports to the relevant MDTs. Support the Clinical Operational Lead and B7 with the development, implementation, and delivery of new clinical pathways to support service growth. Work collaboratively with Sirona and NBT staff organising NHS@Home caseload and managing referrals. Escalateclinical concerns appropriately. To work autonomously making clinically effective and appropriate decisions. Undertake community visits to deliver clinical care.
