We have an exciting opportunity for a suitably experienced Band 6 Occupational Therapist to join our established community therapy team covering the Knutsford Care Community. The role will involve providing comprehensive Occupational Therapy assessment and treatment of adults within the Knutsford Care Community. This may be within patient's homes, nursing and residential homes and within our rehabilitation beds. You will be part of a wider integrated team consisting ofPhysiotherapists, Therapy Assistants, GPs, District Nurses, ACPs, and Social Workers. Hours per Week: 22.5 To act as a clinical specialist providing a high standard of comprehensive Occupational Therapy assessment and treatment service to patients with diverse health needs across the Knutsford area. This care will be delivered in patients homes and within our community bed based settings. The successful candidate will be working as an autonomous clinician, using accurate analytical skills and clinical reasoning to interpret assessment findings, making diagnoses where appropriate, identifying patient problems, setting goals, and providing an appropriate treatment plan and progression. To utilise sound communication via liaison and referral to other agencies as required embracing care community team philosophies. "}
