Main duties of the job The role positively contributes to the managing of ward supporting registered nurses in a provision of care and support ensuring patients receive the highest quality in a safe environment. Main duties include assisting patients with daily living and independent skills - helping people to be organised, maintain hygiene needs and supporting their meal-plan. The role supports patients in aiding communication and providing emotional and behavioural support whilst encouraging patients to be proactive in their care. Some Of The Key Duties For The Role Include Supporting the nursing staff on the ward with the assessment of patients. Perform clinical baseline observations. Assisting in the admission and discharge planning of patients. Work alongside members of the multidisciplinary team, including different teams i.e. physiotherapist, occupational therapist, SALT, Dietician and report any changes in a patient's condition. Record and document episodes of care in accordance with Trust guidelines. Escort patients for investigations within and outside the Trust. Maintain effective communication within the team and between other teams, patients and relatives. Observations, monitoring and recording patients conditions by taking temperatures, pulse, respiration and weight. Checking and ordering supplies. You will work with additional support from the practice development nurses, and ward manager.
