The post is based at Preston West. Working hours are from 07:45-22:00. Working as a member of the team, you will provide holistic, high quality, clinically effective care to patients, working under the guidance and support of the Team Leader and Team Manager. Chorley & Withnell District Nurses- based at Coppull Clinic, PR7 5EJ Community Nurses are highly skilled practitioners who carry out interventions which include:- Holistic assessment of needs Wound care End of life and palliative care Lower limb assessment and management Diabetes management Bladder and bowel care Safeguarding/ MDT working IV therapy Admission avoidance Discharge facilitation Sepsis screening The successful candidate must demonstrate excellent clinical and communication skills to provide a high standard of care to ensure a positive patient experience. Experience in community care is an advantage however the post is also suitable for newly qualified nurses. We have an excellent preceptorship programme which all our newly qualified staff will follow. Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care. The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
