This role has been established with the aim of ensuring that the service to the member is delivered seamlessly, effectively drawing together the work of the Support Team, Physiotherapy, OT, Nutritionists, Counselling, external health and medical professionals and the Charitys Housing/Property Service. The focus of the role being to get it right first time and every time. You will be highly organised and an effective communicator/mediator with an eye for detail and methodical in your approach to any task. You will also possess the skill set to be able to undertake a range of service and property audits, reporting outcomes as appropriate and ensuring remedial action is taken promptly. Working in conjunction with the Senior Support Worker you will ensure that the Staff Team and Professionals work with a common purpose. You will have a can do attitude to every aspect of your work and also be able to lead by example in your PA role being empathic, supportive and outcome focused. Job Purpose To listen to and respect the views, opinions and wishes of the member. Work collaboratively in conjunction with the Support Staff Team, working alongside a member of the Support Team, when the activity requires an enhanced level of personal support. To provide personal care as required. Assist with attending appointments as necessary and ensuring appropriate preparation / documentation is completed in a timely manner. Analyse and review what has happened each week and understand what went well and not, informing/amending future planning accordingly. Participate in organisational training. The successful candidate will be experienced in a similar role within the care sector ideally working with clients with physical and learning challenges.
