The shift pattern for this position is 9-5/MON-FRI. This is an exciting opportunity to be part of our Post Diagnostic Support Team within our Older Peoples Community Mental Health Team (OPCMHT) in East Dunbartonshire HSCP and deliver a minimum of 1 years Post diagnostic Support to people newly diagnosed with dementia. This is in line with the Scottish Government commitment to provide anyone who receives a diagnosis of dementia access to post diagnostic support for at least one year. The post holder will provide advice, information, emotional and practical support to people with Dementia and their carers using the 5 pillar Model of Post Diagnostic Support. The post holder will work within the OPCMHT based within Woodlands Resource Centre. Key Duties Include Act as a named contact for the person and their carer/family To use the 5 Pillar model to work with people newly diagnosed with dementia Provide information, advice and emotional & practical support to people & their carer/family Enable the individual and their family to develop a robust personal plan that enables individuals and their family/carers to identify their own supports to enable them to live well within their own community. Develop a future care plan which includes information about what is important to them and their current wishes around future care. To ensure that post diagnosis support is personalise and flexible which will lead to agreed and specific personal outcomes To review and monitor change and adapt to the persons needs To utilise a health promotion approach and ensure an awareness and understanding of the range of diseases known as dementia. To liaise and collaborate with other professionals involved in providing dementia support and care services Where appropriate facilitate the establishment of short life support groups/Educational groups for carer & families.
