New starters joining the Trust before this time will commence on the old payscales and will move to the new ones once they have been finalised** The Care Assistant – Community role will undertake a broad range of Health and Social Care activities, with a specific focus to, either keeping people safe and well to avoid unnecessary admission to hospital, or to support people to return home and live independently at home following discharge from hospital. Primarily you will work with people in their own homes and communities, often as a lone worker. The role will support the Registered Practitioner in the provision of routine and fundamental health and social care support to people. The post holder will respond to service users’ personal health (both physical and mental health), and social care needs as well as wider independent living skills, such as social isolation, employment or other needs. Acting upon initial referral information but responding flexibly to the person’s presenting needs.
