As a Commissioning Practitioner, you will be responsible for assessing individuals, ensuring the National Framework for NHS Continuing Healthcare and Funded-Nursing Care is adhered to. This also includes planning and commissioning packages of care and the review of these to ensure they meet identified health outcomes. You will support personalised care by offering personal health budgets and help individuals and their families to understand these.
