Due to the rurality of Lincolnshire some areas are not well covered by public transport therefore having access to transport is essential. The service works through the hours of 8:00am to 6:30pm 7 days a week and is covered by flexible working patterns. The Community Nursing service delivers a 12 week induction programme for all new staff. We also offer a robust preceptorship and clinical skills programme with an experienced clinical education team. This provides peer support in addition to the requisite training required to fulfil the role to its true potential. Lincolnshire Community Health Services are seeking registered nurses to join their dedicated teams on a full or part time basis within the Skegness and Mablethorpe Community Nursing Teams The Team is based at Skegness Hospital Dorothy Avenue Skegness As a community staff nurse you will be required to work autonomously and as part of a team, delivering care to patients in their own homes and care homes. Successful candidates will be responsible for assessing care needs, and the planning, delivery and evaluation of nursing care using innovative evidence-based practice. To support you in your new role we have developed an induction programme which allows you to learn the skills and knowledge required to be successful. The induction programme includes teaching sessions in the key areas of practice; Holistic Assessment and Care Planning, Palliative Care Awareness, Wound Care and Medication Administration. Along with your mentor you will work through the key areas of practice at your won pace until you feel confident and capable in undertaking visits yourself. Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Working in this capacity embodies the person centred care approach and allows patients to truly be at the centre of care delivery. The skillset of a community nurse is vast and non-exhaustive requiring knowledge and expertise in a range of areas. Examples of interventions provided are wound management, continence care, palliative care, medication administration and long term condition management. Close working with the MDT (Social Workers, GP's and Allied Health Professionals) to ensure that patients care is effectively co-ordinated, demonstrates a positive impact on our patients.
