Parkfield Medical Centre in Potters Bar, has an opportunity for a care co-ordinator to join our team on a part time basis for 30 hours per week. This is for a 6 month fixed term contract to start asap. You will support the practice in coordinating all key activity including access to services, advice, and information, and ensuring health and care planning is timely, efficient, and patient-centred. If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you. Carrying out audits to identify improvements and maximise income in IIF (Investment and impact fund), ECF, QOF, enhanced services and claiming procedures. Managing the recall system for contract areas such as QOF, IIF, ECF or any other contracted services. updating IT systems to include templates and protocols to meet the requirements of any changes in service specifications, e.g., IIF changes. Working with the GPs and other primary care professionals to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals. Raising awareness within the Practice of shared decision making and decision support tools. Raising awareness of how to identify patients who may benefit from shared decision making and support staff and patients to be more prepared to have shared decision-making conversations. Utilising population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care. Supporting with the DPST toolkit and help to fulfil the elements of the toolkit such as audits. Giving specific support to help plan and deliver efficient Flu and Covid vaccination campaigns using IT tools available to you. Attending and hosting meetings with the local Carers Champions. Devise and maintain a system that will provide carers at Parkfield with up-to-date information and advice. Covering reception and administrative duties. "}
