ADMINISTRATIVE RESPONSIBILITIES Ensure patient records are kept up to date and accurate and recording is done in line with Trust policies and procedures. TEACHING & TRAINING RESPONSBILITIES To attend specifically fire and lifting and handling lectures annually and ensure appropriate updates are attended once H.C.A. training is complete. To participate in an annual performance review undertaken with the Ward Manager to identify personal development needs. An exciting opportunity has arisen for dynamic and motivated registered nurses to join our highly skilled, innovative, and rapidly expanding Frailty service. The post holder will be mainly based on Bevan Court 1 The Frailty Service compromises of: OPAL (Older Persons Assessment and Liaison Service) This new twenty-four bedded unit is geriatrician led and open 24/7 to provide an overnight stay/short stay area for those patients living with frailty who need further review and treatment. A dedicated Ambulatory unit, which provides rapid Comprehensive Geriatric (CGA) for patients with frailty and frailty syndrome. A 16 bed in patient Frailty Assessment ward, that benefits from daily Consultant Geriatrician leadership along with a dedicated team of nursing, therapy, and social care professionals. A Community Frailty Service for St Helens CCG that provides Comprehensive Geriatric Assessment, post discharge from hospital follow up, and provides ongoing management for frail patients in the community. An Urgent Community Response team providing a 2hr response for and urgent health, social or therapy input. Frailty Virtual Wards providing a hospital at home service for those patients living with frailty in the community (own home or care setting). Post holders will also have an extended induction and training programme, prior to commencement in post.
