A key role for the post holder will be to undertake home visits assessed as requiring a visit by their own GP, duty doctor and through referrals reviewed by you directly. Key duties and responsibilities 1. Clinical Practice a) Work as part of the triage team with the Duty GP to assess patients needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis, both through home visiting and as part of clinic based duties. b) Work as part of the Emergency Practitioner Team in the organisation of MDTs for care home patients, both assisting with diagnosis, treatment and management. You will be required to foster excellent working relationships with local care home managers and staff in order to ensure the highest level of care is available to this vulnerable group of patients, with a focus on patient centred care delivery and admission avoidance. c) Clinically examine and assess patient needs from a physiological, psychological and social perspective, and plan clinical care accordingly. d) Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service. e) Order, or provide if necessary, investigations and samples, provide treatment, counselling and health education f) Diagnose and manage acute conditions supported by appropriate prescribing (where qualified). g) Perform safe assessment and management of patients via telephone or face to face. h) For Prescribing Practitioners you will prescribe medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice and antimicrobial stewardship.
