The Clinical Pharmacist will work as part of the integrated practice team and liaise on a daily basis with the clinical and administration team to manage clinical issues as per the duties and responsibilities of the post holder. The Clinical Pharmacist will have a key role in ensuring the safety and effectiveness of the prescribing at the practice, by working with the repeat prescription team, care homes and other healthcare professionals in a number of roles, including education and prescribing in their own right. This will include structured medication reviews for patient with long-term condition or on high-risk medication, active participation in weekly care home board rounds and responding to medication queries from patient and health care professionals. Reviewing and updating current protocols and procedures will also be an important part of the role. The post holder will work as part of a multi-disciplinary team in a patient-facing role.The post holder will provide primary support to general practice staff with regards to prescription and medication queries. Provide expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).
