The Advanced Practitioner will provide an expert level of clinical assessment and support to the care and treatment of a diverse and undetermined caseload of patients within Emergency, Urgent and Unscheduled care. The practitioner will achieve this by using their in-depth knowledge, experience and broad range of techniques, to assess, investigate, interpret, diagnose, treat and refer or discharge patients. The successful candidate will work on a Tees home visiting Contract, as part of a new Integrated Care Service. Bases will be urgent treatment centres in: North Tees, Hartlepool, South Tees and Redcar. Hours are 18:30 - 08:00 7 days a week ( shifts are 6:30pm to 6:30 am or 8pm to 8am). On weekends and bank holidays 24 hour cover is needed (shifts are 6:30 am to 6:30pm, 8am to 8pm or 10 am to 10pm.) For queries contact - Sean.potts@neas.nhs.uk The Advanced Practitioner is an experienced clinician who can demonstrate a high level of autonomous practice and complex decision making. This is underpinned by Masters level education or equivalent (or RCN Accreditation), covering the four pillars of advanced practice; clinical practice, leadership and education, research, education. This senior clinician will provide an expert level of clinical assessment and support to the care and treatment of a diverse and undetermined caseload of patients within Emergency, Urgent and Unscheduled care. The Advanced Practitioner will undertake clinical assessment including history taking and physical examination, form a diagnosis and provide a treatment plan, which may include prescribing. Patient assessment may involve both telephone and face to face consultations. They will work across a variety of settings offering first point of contact at an appropriate place and time. This may include an emergency response to the patient in their own home or other community setting. "}
