The post holder will support qualified team members (Community Children’s Nurse, Physiotherapist, Occupational Therapist) working under their guidance and supervision in carrying out a person centred plan of care, including children with complex care needs, within the family home setting. It is expected that the post holder will work independently with patients in the community and within the agreed parameters of a defined care plan and an agreed competency framework. This may be in a patient’s home, community hospital or any other appropriate setting. They will work as part of the multidisciplinary team, enabling people to achieve their optimal level of independence utilising reablement/enablement programme of care and supporting the individual to remain in their own home and where appropriate deliver care as directed by the professional. Night-duty working is integral to this post. They will assist in providing nursing care for those who are experiencing short term acute illness, exacerbation of long term conditions or those wishing to receive end of life care within their own home.
