The post holder will develop positive working relationships with networks of health and care professionals, working in a person-centred way to support the wellbeing of their clients. The post holder may support clients with fully health funded, or joint health and social care needs who require specialist health input. They will be actively involved in developing care plans and advising on appropriate packages of care in the community or supporting with stepping down from an inpatient facility. They will ensure high quality care is delivered that is effective, safe; and clients and their advocates experience positively. Main duties of the job 1. Case Management and Case management Reviews 2. Undertaking Robust DST assessments 3. Completing CHC Reviews 4. Working collaboratively with the team and doing duty 5. Effectively provide quality and commissioning case input and oversight to clients with a learning disability and complex care needs. 6. Plan, implement and evaluate care plans for clients and their carers using evidence based and person-centred principles 7. Provide specialist nursing advice and support to clients with a learning disability and complex care needs, their families, or other providers, clinicians or professionals involved in the care, using evidence based practice 8. Liaise and work in a joined-up way with the client, their family, relevant health and care professionals, and all other health and social care agencies involved in the care of the person. Evidence of CPD relevant to role Educated to degree Level Desirable RNLD 5 years post senior clinical experience.
