The Band 6 post holder will manage a defined caseload, use evidence based/patient centred principles to assess, plan and deliver individual treatment plans. Main duties of the job Ealing Community Partners: NHS, Ealing Council, voluntary and specialist providers working together to deliver health and social care for the people of Ealing Community-based health care in Ealing has undergone a transformation. Detailed Job Description And Main Responsibilities To autonomously assess and diagnose patients with a diverse range of musculoskeletal, orthopaedic or rheumatology conditions, using appropriate assessment/diagnostic techniques, skills and tools To adopt a problem solving approach, engaging patients and carers in their treatment plan, negotiating on agreed outcomes, gaining consent and educating users on self-care. To evaluate and review treatment outcomes to facilitate discharge of patients. To deliver care in a range of settings including primary sites, clinics, GP surgeries and acute hospital as required. To work with members of the wider multidisciplinary team to ensure a seamless delivery of care, provide advice and education within this forum, acting as an expert in your own professional field. Orthopaedic out-patient experience as a band 5 Awareness and understanding of Audit and Clinical Governance Attendance on relevant MSK courses Broad under graduate experience in core clinical areas: MSK/ Respiratory/ Community Therapy/ Neuro/ Care of Elderly.
