We are recruiting for a Healthcare Assistant to join our community-based team in Somerset. In this role, you will work alongside a multidisciplinary team to provide assessment, care planning, review and evaluation of holistic care management plans individual to each patient. As this is a community-based role, you will be responsible for planning your own work, responding to changes in patient conditions, prioritising care needs (including hygiene, nutrition and hydration, comfort, monitoring vitals, and more) as detailed in the personalised care plan and ensuring you escalate appropriately to senior staff within the multidisciplinary team in a timely manner. What were looking for: National Vocational Qualification (NVQ) Level 2 in Health and Social Care OR an equivalent alternative Level 2 Qualification. - Minimum of 6 months care delivery experience in a Health or Social Care Environment.
