Supporting carers and families is essential work. A core principle is that the YOD service provides continuity of care. The service engages with people from the point of diagnosis and remains involved across the entire course of an individual’s dementia progression. The service is founded upon developing collaborative relationships of trust, understanding individual’s personal stories, bringing people together to enjoy mutually supportive social engagement activities, and being available to respond to often rapidly changing mental health, psychological, physical and social care needs. The service is based at the Cariad Centre, Barry Hospital, which is alongside the services inpatient facility at St Barrucs ward. This is a fantastic development opportunity to work in the community broadening skills in holistic assessments of the needs of clients, carers and families. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. To provide specialist nursing assessment, evidence based intervention, advice and support for service users and carers and families as appropriate to specialist area of work Provision of advice and support to professional colleagues on care management of people with dementia and advice and support to care staff on person-centred interventions Assist with the implementation and audit of the Dementia Care Advisor role, to support further role development at UHB and All Wales evaluation Assist with delivery against the Dementia Intelligent Targets, supporting development of appropriate clinical care pathways, development and delivery of specialist training, undertaking audit activity and outcomes reporting against key performance indicators Participate in, make effective use of and provide clinical supervision to enhance own performance and that of colleagues.
