We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries . The Discharge To Assess element is multi-professional services who assess patients upon discharge from hospital in the community .We provide individualised care to meet the person’s needs and facilitates transfer for onward care into the appropriate service: Health, Adult Social care or partner agencies. The Rehabilitation Element This element incorporates a free, short-term rehabilitation service for adults to help prevent unnecessary hospital admission, enable early discharge from hospital and prevent unnecessary admission to long-term residential care. The service is for adults who have been assessed as medically stable and who do not require hospital care. To undertake tasks which involve the management and maintenance of the teams clinical caseload for Senior Support Workers, including all associated administration tasks. The job will include using a variety of computer based programmes including EMIS, Advantis and Microsoft Packages. Liaise with different Professional's to coordinate the patients care. Responsible for delegation of visits and coordination of 15 support workers on a daily basis.
