The role will require clinically effective assessment, care management skills and interventions in accordance with the Care Programme Approach and Care Act framework. The post holders duties will include engagement with clients in the development of person centred, recovery focused care plans, supporting our experienced duty team with short-term pieces of work and supporting people managing crisis. You will need to demonstrate excellent communication skills and be able to work collaboratively in a multidisciplinary setting. You will be dedicated to delivery, both as part of a team and working independently, of a high-quality, innovative, recovery focused and person centred care which meets the needs of people who use our service and their carers. Main duties of the job Main Duties of the Job The successful applicant will work effectively with adult service users with mental health needs across Bury. The post holder will be highly skilled and competent in both holistic health assessment and identifying social needs, risk management and Formulation, developing collaborative care plans, facilitating the implementation of the agreed plan for care with a focus on recovery and social inclusion. The post holder will provide a range of interventions and the opportunity to develop these within the role.
