They will take responsibility for the management, including supervision and appraisal of Band 6 Care Coordinators ensuring the delivery of high quality, evidence based practice. They will coordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members and ensure effective interface and continuity of care between disciplines and services. They will also be responsible for the development and maintenance of positive professional relationships with housing, social care and third sector colleagues in the relevant borough.
