You will be connected with peers across the CIS tri-borough of Hammersmith & Fulham, Westminster and Kensington and Chelsea as well as West London NHS Trust Working as a member of the Community Independence Service / Case Management Team you will supported by registered allied health clinicians and nursing staff you will to deliver skilled health and social care to people in their homes; the care you deliver will be based on delegated tasks by a senior clinician following thorough holistic assessment of person’s clinical and personal needs and their agreement of an individualised care plan. The care you deliver will be within the boundaries of your role, focusing on supporting people with a range of complex needs and long term medical conditions (mental and physical). The aim is to reduce the impact of health inequalities, improve access to health and care services, address complex health, social and housing needs, reduce social isolation and optimise wellbeing.
