This role will primarily support the Community Health Pathways Programme and inform delivery across other programmes such as Dermatology, ENT, General Surgery, Gynaecology, Ophthalmology, Orthopaedics, Urology, Cancer, Outpatient Transformation, to transform future service models for planned care, and champion best practice and a ‘Once for Wales’ approach. The Primary Care Clinical Lead for Strategic Programme for Planned Care will be expected to provide expert clinical advice to inform the planning and delivery of sustainable planned care services and be a source of expertise for Health Boards, regions, and nationally advising Welsh Government where required. We would welcome applications from colleagues with innovative thinking and the necessary skills and experience, as well as the ability to collaborate and lead the delivery of the immediate priorities and objectives of the National Planned Care Plan. The Post Holder Will Work In Partnership To Develop And Deliver The Aims And Objectives Of The Strategic Programme For Planned Care Provide national clinical leadership and professional direction in developing national clinical pathways, ensuring these systems can be adapted and implemented locally by Health Boards. Work in partnership with the Strategic Programme for Primary Care to maximise the opportunities of the Primary Care Model for Wales for planned care. Work with system partners to systematically embed prevention across and within all health and care pathways Collaborate with stakeholders within Health Boards, NHS Executive and external partners (such as GPC Wales) to ensure the successful implementation of effective pathways Establish and maintain a robust Quality Assurance process for clinical pathways. Support pathway development at local, regional, and national levels. Report on the progress of national and local clinical pathway development to relevant stakeholders. Lead the implementation of “advice and guidance” across NHS Wales, fostering a Welsh network and actively supporting specialties to integrate specialist advice and guidance into the referral pathway. Support the evaluation of pathways and advice and guidance systems.
