We are a dynamic, friendly practice in the heart of Aldershot serving a diverse population of 12500 patients. We have a strong focus on GP training and medical student supervision. We are expanding our team to recruit a Mental Health Practitioner. The MHP will be responsible for providing comprehensive mental health specialist care within the organisation. You will be expected to develop, plan, coordinate and deliver the service whilst working as part of the organisations multidisciplinary team. The MHP will work as an autonomous practitioner and be responsible for the provision of assessments, interventions and education. They will have responsibility for managing a caseload of patients with a variety of mental health conditions and will be expected to collaborate with other healthcare professionals both internal to, and external from, the organisation. Communicate proactively and effectively with all colleagues across the MDT, attending and contributing to meetings as required Work with patients to support shared decision-making about self-management, facilitate onward access to treatment services and provide brief psychological interventions Work closely with other PCN-based roles to help to address the potential range of biopsychosocial needs of patients Complete comprehensive risk assessments informing outcomes and positively managing risk indicators Provide a range of therapeutic interventions. Maintain accurate and contemporaneous records When necessary, provide support, information and training to family members, carers and other professionals Take the lead role in undertaking comprehensive initial health and social needs assessments Operate without the need for a formal referral from GPs, including accepting some direct bookings where appropriate About us Job Summary: To be responsible for providing comprehensive mental health specialist care within the organisation.
