To work as an autonomous practitioner using advanced clinical reasoning skills to provide specialist therapy advice to the ToCH and support correct discharge pathways. Promote awareness of the role of Occupational Therapy within the ToCH and with the discharge process. To provide acute therapy representation at the daily system capacity huddle meeting and progress any necessary actions for therapy. To work in partnership with the ToCH to pro-actively deal with queries related to OT assessments, equipment provision or any therapy issues that may be perceived to be causing a delay to a patients discharge. To be involved in clinical practice issues, raising the profile of the service and offering specialist advice to planning and service development in order to assist the team leader/service manager in the implementation of service improvements. To work towards improving quality services including implementing evidence based practice. To work toward the achievement of specific response time targets. To complete effective, concise documentation of patients records in line with Trust, professional & department standards. To ensure accurate data entry takes place in line with departmental standards. To participate in the clinical education of staff within the specialism and be responsible for the supervision of unqualified, newly qualified therapists and undergraduates who work under your direct supervision. To participate in the service and patient pathways in line with clinical standards and best practice. To participate in research activities pertinent to the role. To assist in raising the profile of the service. We are looking to recruit two Band 6 Specialist Occupational Therapists to be part of a new role within the acute OT inpatient teams and the Transfer of Care Hub at Sunderland Royal Hospital. This is an exciting opportunity to work with dedicated multi-disciplinary and therapy teams to aid effective and efficient discharges from the hospital environment into the community while working to the national principles of Home First and Discharge to Assess. These posts will rotate between the Transfer of Care Hub and the acute medical and surgical inpatient OT team to allow you to apply your knowledge, skills and experience while still being involved in the direct delivery of Occupational Therapy assessment and intervention. These posts are jointly supported by the Directorate of Therapy Services and the Community 1 Directorate of the Trust and will be part of the acute adult Occupational Therapy team based at Sunderland Royal Hospital. The team is large and friendly and works closely with other professionals within and outside of the hospital in both health and social services where we place great importance on the benefits of partnership working for the people of South Tyneside, Sunderland and surrounding areas. You will need to have experience of working with acute hospital patients, which could be through the acute inpatient phase leading up to discharge or with patients as they first leave the hospital environment and have working knowledge and experience of complex moving and handling assessments, equipment prescription and assessment for care needs. You will also need to have excellent communication skills to engage with patients, carers and other services and teams, as well as the ability to effectively prioritise and manage your time. The posts are Monday to Friday, 37.5 hours per week with a requirement to work some bank holidays. We offer a flexi time system and actively support CPD opportunities.
