The Role: Healthcare Assistant/and reablement On a Day-to-day Basis, Youll Attend 45-minute (minimum) patient visits. Offer personal care, including meals, personal hygiene rehabilitation supportand overall reassurance and safety. Monitor and record conditions and alert the Clinical Lead to any changes. Collect and transport clinical specimens. Observe pressure areas and skin integrity, and report changes to a qualified nurse. Provide additional support with training if required in the following areas: reablement, phlebotomy, holistic support. Youll visit patients in their homes, carrying out nursing and rehabilitation tasks under the direction of a qualified practitioner. We require Rehabilitation Support Workers to work seamlessly alongside our clinical teams to provide rehabilitation and reablement services to patients in their own homes to help support their recovery following illness / admission to hospital. About You Alongside a relevant NVQ Level 2/3 or equivalent qualification, youll have the confidence to work autonomously in the community and share our commitment to delivering quality patient care. Compassionate and collaborative, youll bring with you strong communication skills and the ability to build effective relationships with patients, carers and colleagues alike. Youll also need: The right to work in the UKwithout any form of sponsorship A drivers licence and access to a car, this is essential as its a mobile role.
