The aim of the team is to triage and assess patients that come into the emergency department by ambulance, with a view to preventing admission to hospital. The post holder will work within the A&E department based at the Royal Sussex County Hospital. The post holder will act as an ambassador for Sussex Community NHS Foundation Trust and play a key role in supporting at the front door of A&E in preventing hospital admissions. Utilising their knowledge of the community services and clinical skills the post holder will work closely with the A&E staff to ensure seamless, safe and effective discharge plans are in place for the person to return back to their usual place of residence as soon as possible, preventing admission to hospital. The post holder will play a key role in using data from incidents, feedback and observation to identify potential improvements or changes within the service which will support effective safe discharge We are the main provider of NHS community services across East and West Sussex.
